How do I schedule an appointment with Bon Secours Physical Therapy?
If you are reaching out from a physician’s office, please call or fax the patient referral to the location most convenient for the patient. Our friendly office staff will assist you with scheduling so your patient can be seen in a timely fashion. When faxing a prescription, be sure to clearly include the patient’s name and phone number, so we can contact them and get them scheduled as soon as possible. We will assist you with insurance authorizations. A personalized Patient Plan of Care will be faxed to your office after the first evaluation has taken place. Progress notes are sent before the next doctor’s visit.

If you are a patient who is interested in signing up for a program, please contact us or your preferred clinic location directly.

Can I fill out my forms before my appointment?
We encourage our clients to save time on their first visit by downloading and filling out the necessary patient forms in advance. Everyone should complete the outpatient registration, medical history and personal representative forms. Please bring the completed forms with you to your first appointment. Download Bon Secours Physical Therapy patient forms.

Where do I learn more about my physical therapist?
Visit our Bon Secours Physical Therapy Team page to view our staff bios, credentials and photos and to learn more about the team members you’ll be seeing!

Do I need a referral to schedule an appointment?
Not necessarily.  Direct access to physical therapy (PT) is your opportunity to be evaluated and treated by a licensed physical therapist without first seeing your physician for a referral. Learn more about Direct Access and how it can give you quicker access to your treatment.

How do I cancel or reschedule an appointment?
If you have an existing appointment with a Bon Secours specialist that you must cancel or change, please call your clinic directly. No schedule changes can be made through the website at this time. We request at least 48 hours’ notice for canceled appointments.

Are you hiring for physical therapy positions?
Yes! At Bon Secours Physical Therapy, we love meeting talented people. If none of our open positions meet your qualifications, we still encourage interested applicants to submit an application and resume to be kept on file for future consideration in physical therapy and sports performance job openings in Richmond.
